Urgent survey


Please could you take two minutes to complete our survey. This is quite an urgent request, as all surveys need to be completed no later than THIS SUNDAY (March 25th). Your help is much appreciated and the information gathered will help improve FASD awareness and services.

FASD Awareness South East is conducting a survey to identify levels of FASD understanding and support, within the UK healthcare system.Evidence is needed to back a proposal for a FASD pathway, please if you could fill in this survey it would be a great help. This survey should take less than two minutes to complete.
